Who can register as a
patient
Registered Patients
People who are normally resident within the Practice
catchment area of Poynton, Hazel Grove, Adlington,
and
parts of
Prestbury, Pott Shrigley and Woodford
may apply for inclusion in the list of registered patients of
Priorslegh Medical Centre.
Temporary
Registration
In
addition, people who are away from their normal place of residence,
and are temporarily residing within the Practice catchment area,
(over 24 hours but less than three months), may apply for temporary
registration providing they are not receiving essential services
from any other primary care provider in the locality.
How to Register
as a Patient
If you would
like to register as a patient with Priorslegh, and you live within the
catchment area, present your medical card at Reception and ask for a
patient registration form and
a medical questionnaire. You will be asked to make an appointment with
the Practice Nurse for a new patient medical.
Removal from the
list of registered patients
A
Patient may be removed from the list of registered patients at
Priorslegh Medical Centre if he/she:

requests removal and registers
with another medical practice
-
no longer resides within the
designated Practice catchment area
-
is absent from the UK for a
period exceeding three months
-
displays violent, abusive or
unacceptable behaviour towards any member of the Practice team,
or whilst on the Practice premises.
