Useful Information
Disabled Access
Priorslegh Medical Centre
has been purpose-built for easy access. Most of the patient facilities are
on the ground floor; there are toilet facilities for the disabled; height
adjustable couches and
a stair lift for access to upstairs facilities.
Please
let
the receptionist know if
you wish to use
these
facilities.
Medical Certificates
For the
first 3 days of any period of sickness absence from work you do not
need a sick note. A self-certificate (SC.2 available from Reception)
should be completed to cover all periods of sickness absence of 4 to 7
days; a doctor’s certificate (MED.3 or 5) can only be provided for
periods exceeding 7 days. If you need a certificate to cover a period
of less than seven days, a private sick note can be provided for which
a charge is payable.
Practice Catchment Area
The Practice catchment area covers
Poynton, Hazel Grove, Adlington,
and
parts of
Prestbury, Pot Shrigley and Woodford.
If you change your name, address or telephone
number, please inform reception at the earliest opportunity. A change
of address may mean that you are outside the practice
catchment
area and may
need to change your GP. If in doubt please check with the
Administration Manager.
Transport
by Ambulance
If transport by ambulance has been approved on medical grounds, this can be
arranged via the receptionist (01625 872299). Please give three working days’ notice for routine requests.
The
local voluntary group ‘Open Hands’ (Tel. 01625 878589,
10.00am - 12 noon, weekdays)
can
often
provide transport to
the surgery for patients unable to attend by their own means,
and also to Macclesfield and Stockport Hospitals .
Please give as much notice as
possible.
A
donation for petrol is always welcomed!
A fee
may be
charged by the Ambulance Service for transport to other hospitals
or clinics.
Chargeable Services
Not all the services
provided
by the Practice are covered by the National Health Service. A list of
chargeable services is displayed in the waiting area together with
appropriate fees and charges. All payments for private services must
be made in full BEFORE the service can be provided. Please note that
payments by cheque can only be accepted with a valid cheque guarantee
card. We regret that we are unable to accept payment by debit or
credit cards. Appointments for private medical examinations, cannot be
made for the doctors’ normal surgery times.

Friends of Priorslegh is a registered charity run by a group of patients
to raise
funds
for the surgery.
The group’s hard work has provided additional medical and other equipment
for the surgery.

at reception for details.
